About the position Responsibilities • Developing a community support plan (goals) for each individual, typically managing a caseload of 40-45 individuals. • Assisting individuals in accessing services and developing service agreements and documentation through case notes. • Informing individuals or their legal guardians about service options that fit their needs. • Meeting one-on-one with each individual at least twice a year to evaluate progress and adjust care plans as needed. • Identifying potential providers to guide individuals towards their goals. • Evaluating and monitoring the services identified in the care plan. • Ensuring compliance with case management rule 185 and MN Health Care Programs regarding waiver services. Requirements • Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field. • If holding a Social Work degree, must be currently licensed as a Social Worker as required by the Minnesota Board of Social Work. • Bachelor's degree in any other field with at least one year of experience working with individuals with a mental illness or disability. • Good written and oral communication skills. • Ability to work independently and handle crisis situations effectively. • Comfortable managing high-volume paperwork. • Attention to detail and process-oriented mindset. • Able to pass a DHS background check, possess a valid driver's license, and maintain a clean driving record. • Reliable transportation.
