UHS is looking for a Virtual Health Provider - Nurse Practitioner (NP) to join our cutting-edge Hospital Care at Home program. In this role, you will provide essential care to a high-acuity patient population, including those recently discharged from the hospital, ED, and walk-in clinics. Your expertise will be crucial in preventing readmissions and empowering patients to heal in the comfort of their homes.

You will provide care to patients in their homes using telemedicine as part of a team-based model of care which will at times include community paramedics.

You can either work remotely or in a hybrid model.

You must be board certified/eligible and licensed to practice in New York State. Must have completed an accredited Family Medicine NP Program.

Salary Range: $126,300 - $148,600 annually
