As a Virtual Care Coordinator, you will play a crucial role in empowering individuals to take control of their health and wellbeing. Key Responsibilities: Conduct virtual phone conversations to manage patients' healthcare needs and develop tailored care plans addressing physical, mental, and preventative health challenges. Utilize cutting-edge technology platforms and Electronic Health Records (EHRs) to document patient visits and ensure seamless care coordination. Coach patients on treatment plans, including nutrition and wellness strategies, to promote holistic health and wellness. Connect patients with essential resources and prepare them for medical appointments to ensure comprehensive care.
