Virtual Bed Coordinator Registered Nurse – Pulse Center – Per Diem, NY

Virtual Bed Coordinator Registered Nurse - located in Big Flats, NY! This position is per diem. Position Summary: The Bed Coordinator works with the Guthrie system to place patients in beds based on patient need and level of care Education, License & Cert: • Dually licensed in NYS and PA • BSN strongly preferred • Certification preferred Experience: • 5 years of clinical experience in acute care setting required and 1 year in Critical Care strongly encouraged. • Demonstrated leadership (minimal charge nurse role) and effective communication required. Essential Functions: • Seeks to support a professional practice environment which fosters excellence in nursing practice. • Fosters professional nurse empowered decision‐making, accountability and autonomy in nursing practice. • Oversee and assist with the placement of patients using the right patient, right unit methodology • Maintains confidentiality while serving as a professional liaison with the community. • Participates in professional continuing education programs. • Participates in nursing committees, performance improvement activities and initiatives. • Demonstrate effective communication, written and verbal • Always demonstrate professionalism and respect • Provide detailed handoff to oncoming Bed Coordinator • Deliver updates to onsite leadership for situations or events to ensure appropriate follow-up. • Escalate situations to the appropriate onsite leadership for direction, support and backup as needed. • Provide conflict management techniques in emotionally charged situations to de-escalate, prevent increased escalation and potential harm • Complete all required documentation needed for validation, improvement, and data collection • Collaborate with the Entity House Supervisor for proper placement of patients. • Guide patient placement for right patient, right entity, right unit • Prioritize the data and information collected based on the patient’s immediate condition or anticipated needs. Work through potential risks or barriers to getting the patient to their destination in a safe and timely manner. • Working with the Entity House Supervisor, coordinate and arrange emergent and non-emergent patient transfer. • Assist with the efficient and appropriate patient bed assignments while monitoring on-going bed availability. Coordinate patient flow throughout each entity while collaborating with the Entity House Supervisor, leadership, administration, and staff. • Assist with inputting patient placement in Epic, when needed • Assist with Inputting transfers in Epic, when needed Other Duties: It is understood that this description is not intended to be all inclusive, and that other duties may be assigned as necessary in the performance of this position.
