UM LPN Care Review Clinician

If you have an active LPN license and a minimum of 2 years of clinical experience, we want to hear from you! Conduct comprehensive assessments of member healthcare needs.
Ensure appropriate and timely care is provided to members.
Collaborate with healthcare providers to develop and implement care plans.
Monitor and track member progress and make necessary adjustments to care plans.
Communicate effectively with members, caregivers, and healthcare team.
Utilize clinical knowledge and judgment to make sound decisions.
Adhere to all relevant healthcare laws, regulations, and guidelines.
Maintain accurate and up-to-date documentation of all interactions and interventions.
Participate in continuous education and training to stay updated on best practices.
Advocate for the best interests of members and ensure their needs are met.
Maintain confidentiality and HIPAA compliance in all aspects of the job.
Collaborate with other members of the healthcare team to provide coordinated and comprehensive care.
Demonstrate empathy and compassion towards members and their families.
Stay informed about industry trends and advancements in healthcare.
Continuously strive to improve processes and outcomes for members.
