CO License
1 Year experience
AHA BLS
Bachelor's Degree in Occupational Therapy
Master's Degree preferred
Previous work experience providing direct patient care in outpatient, inpatient or inpatient rehab unit preferred
Prior use of electronic medical records (EMR) preferred
Other Details:COVID-19 Initial Shot(s)
All workers, including remote, must be fully vaccinated.
Fully vaccinated is defined as 2 weeks after the completion date of a
o 1 or 2 dose monovalent vaccine series, oro 1 dose bivalent vaccine
Exemption for Medical or Religious are reviewed by the client and must be completed using the Centura COVID Exemption form located in Egnyte.
Pfizer, Moderna, J&J, AstraZeneca and all World Health Organization accepted vaccine manufacturers are acceptable.
