We are seeking a compassionate and skilled Nurse Practitioner to join our Optum Home-Based Medical Care team in Pitt & Lenoir Counties, NC. As a valued member of our interdisciplinary team, you will play a vital role in delivering high-quality, patient-centered care to complex, chronic patients in the comfort of their own homes.

Key Responsibilities:

Conduct comprehensive assessments and develop personalized care plans for patients
Collaborate with primary care physicians, interdisciplinary team members, and other healthcare professionals to ensure seamless care coordination
Provide ongoing care and support to patients, including performing routine medical procedures and prescribing medications as needed
Participate in ongoing meetings and quality initiatives to ensure the highest standards of care and promote clinical excellence
Stay current on relevant medical and nursing research, technology, and related issues through ongoing education and professional development

Requirements:

Completion of Nurse Practitioner accredited program
Current NP license and state RN license
Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
Access to reliable transportation and compliance with Optum Motor Vehicle Safety policy

Preferred Qualifications:

2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
Knowledge of CPT, ICD-10, and HCPC codes
Knowledge of clinical standards of care and NCQA requirements
Awareness of CMS guidelines, Medicaid/Medicare contracts, and benefit systems
