Job Description
We're looking for a dedicated and empathetic CareCoach to provide patient-centered care, support care plans, and coach individuals on maintaining a healthy lifestyle. As a CareCoach, you'll:
Provide empathetic support and education to patients with chronic conditions
Develop strong relationships with patients through regular communication and follow-ups
Coach patients on healthy lifestyle choices, including diet, exercise, and medication adherence
Encourage and guide patients in using vital monitoring tools to manage their health effectively
Coordinate patient care across multiple providers and services to ensure comprehensive care
Work efficiently with various Electronic Medical Records (EMR) systems, including EPIC, eClinicalWorks, and Practice Fusion
Requirements
Certified Medical Assistant (CMA) certification required
Experience in chronic care management and patient education
Strong understanding of various chronic conditions and their management
Proficiency in using EMR systems
Excellent communication and interpersonal skills
Ability to work independently in a remote setting
Passion for patient care and curiosity about advancements in AI technology
Bilingual proficiency in Arabic or Spanish strongly preferred
