Telephonic Nurse
Job Summary
EHRMedBilling is seeking an LPN to join our team. The LPN will work closely with patients enrolled in Medicare’s Remote Patient Monitoring and Chronic Care Management programs to improve health outcomes for high risk and multiple chronic condition patients.
Responsibilities
• Create, Update and Review patient Care plans based on providers plan on care
• Make monthly telephonic check -ins with patients to review providers plan of care, assess health and medication adherence
• Assist with care coordination - scheduling, follow ups, updating patient care plans, etc.
• Review RPM synced vital signs and escalate to provider if needed
• Document all interactions in EHR and ensure compliance with Medicare for CCM and RPM guidelines

Requirements
• Active LPN License
• Excellent verbal and written communication
• Experience using EHR systems
• Ability to work independently with minimal supervision
• Reliable internet and quiet/confidential workspace
