Role Overview:

Key Responsibilities:
Perform remote patient assessments, triage, and symptom management.
Provide education on chronic disease management, wellness, and preventive care.
Coordinate referrals, follow-ups, and care plans with on-site providers.
Maintain accurate telehealth documentation and comply with privacy regulations.
Qualifications:
Registered Nurse license in California.
Experience in telehealth or home health nursing.
Excellent communication, technology literacy, and assessment skills.
