About the role We are seeking a dedicated and experienced Support Coordinator to join our team for a permanent full-time role. This hybrid opportunity focuses on visiting participants in Casula and surrounding communities while enjoying the flexibility of working from home. As a Support Coordinator you’ll empower individuals with disabilities to navigate the NDIS and connect with the right services to achieve their goals. You'll work closely with clients, families, and service providers to coordinate supports that truly make a difference. What You’ll Do • Support clients and families in accessing and coordinating services. • Build relationships with internal and external stakeholders. • Implement supports outlined in NDIS plans. • Help clients develop skills to coordinate support and engage in the community. • Empower clients to understand and implement their plans. • Be responsive to client needs, especially in crises. • Conduct regular progress reviews and report to the NDIS.
