Social Worker (Remote) – Geriatric Care Manager

We are hiring social workers (LMSWs / LCSWs / RNs), passionate about older adults, to operate full-time autonomously in a remote environment in a client-facing role:
1099, full-time
• Manage your own client load autonomously: helping families navigate complex systems, coordinate care, and advocate for what their loved ones need
• Pay: $40 - $60 / hour
• Benefits: Flexible, set your own hours (30+ hrs / week, ideally full-time)

• Are tech-savvy and excited to work in a fast-paced, early-stage environment
• Hold an LMSW or LCSW (clinical license is a plus but not required) or a RN
• Have experience in care management, hospice, palliative care, or geriatrics
