Social Worker/ Community Resource Navigator
We are looking for a Social Worker/Community Resource Navigator to join our Transitions Supportive Care (TSC) Team!
Your main responsibility will be to facilitate coordinating care with community resources, educating patients and families about their care plan and the available programs and support systems.
You will work collaboratively as a part of the multidisciplinary team to provide education and support to patients, families, and members of the TSC team.
You will focus on the 8-county service area, with the ability to travel throughout the following counties:
Jefferson, Madison, Taylor, Leon, Gadsden, Liberty, Wakulla, Franklin
Work Schedule and Hours: Monday through Friday, 8 AM - 5 PM in the office.
Travel Requirements: The role requires travel to patients' homes for visits as necessary.

Your Main Duties and Responsibilities
(Includes but, not limited to)
Supervisory Duties:
? Serves as the supervisor for interns assigned to Transitions Supportive Care.
? Provides training on administrative duties associated with assessments, benefit application, and connecting caregivers with resources and services.
? Monitors performance and prepares required intern evaluations and reports to be submitted to education institutions.
Day to Day Responsibilities:
? Assists patients and caregivers understand available resources.
? Maintains updated, diverse resource list for service area.
? Contacts assigned patients/caregivers of 7 Oaks within 24 hours of referral by clinical team. Note: Referrals sent on weekends will be responded to on next business day.
? Offers an in-person visits with caregivers and patients but allows an electronic meeting or email correspondence if family so chooses.
? Provides caregivers with appropriate materials regarding self-care and effective coping with a loved ones illness.
? Follows-up with phone call or email to caregivers/patients within 24 hours of initial contact.
? Contacts caregivers routinely as determined by the patients acuity level to offer support.
? Schedules additional in-person or phone visits at caregivers request.
? Keeps detailed records of contact with and information given to caregivers/patients.
? Maintains strict confidentiality of patient/caregiver information.
? Identifying and coordinating the needs of chronically ill patients and families as well as supporting the clinicians to help them operate as efficiently as possible.
? Communicates with patient/caregiver and community service providers to coordinate services.
? Works in cooperation with the family/caregiver and IDT to meet the emotional and spiritual needs of the patient and family/caregiver.
? Supervises and coordinates training for MSW interns, which includes a weekly supervisory meeting, and the completion of the intern(s) mid-term and final evaluations.
? Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
? Actively participates in quality assurance / performance improvement teams and activities.
? Other duties as assigned.
Education Requirements and Experience:
? Masters in Social Work Required
? A minimum of three (3) years experience working/assisting patients and/or caregivers coping with a serious/advanced illness; connection to and awareness of community resources.
? Documented experience providing social work services appropriate to the casework needs of the seriously ill.
? Prior experience supervising students and interns.
? Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
