About the position

The Adult Protection Social Worker (Intake, Assessment, and Case Management) at Dakota County is responsible for screening, assessing, and coordinating services for Vulnerable Adults identified as needing protective services. This role involves engaging with community resources, conducting interviews, and ensuring the health and safety of clients through effective case management and collaboration with various stakeholders.

Responsibilities
• Engage in follow-up on reports of alleged abuse, neglect, or financial exploitation of Vulnerable Adults.

• Interview reporters and collaterals to gather critical information for assessments.
,
• Mobilize community resources to provide protective services.
,
• Coordinate data sharing with Law Enforcement and Lead Agencies.
,
• Assess and monitor quality assurance of services according to service plans and assessed needs.
,
• Engage and collaborate with medical/behavioral health providers and law enforcement.
,
• Assist Vulnerable Adults in identifying and applying for needed benefits and services.
,
• Complete required documentation for all persons assessed and upon transfer/closure of services.
,
• Attend regular meetings and trainings and participate on work teams as requested.
,
• Support community emergency response functions as needed.
,
• Demonstrate a commitment to cultural diversity and support the County's inclusion initiatives.

Requirements
• Bachelor's degree in Social Work, Psychology, Sociology, Human Services, Special Education, or a related field.
,
• One year of full-time professional experience or two years of full-time para-professional experience in a human service agency.
,
• Valid Driver's license.

Nice-to-haves
• Master's degree in a related field.
,
• Prior professional experience in home or community-based settings.
,
• Knowledge of or experience coordinating Home and Community Based Service Waivers.
,
• Knowledge of or experience coordinating MN Public Assistance Benefits.
,
• Experience working with Adult Protective Services.
