Seeking RNs & LPNs in Charlottesville, Chesapeake, Dumfries (Woodbridge), Suffolk, Virginia Beach, Williamsburg and Virginia Beach to join our local teams in Hospice, Home Care, and Infusion.
Sentara Home Care Services provides a full spectrum of home care services to meet the individual needs of patients. In the past, many of our patients would have had to remain in the hospital for therapy. But today, the highly skilled team of clinicians with Sentara Home Care Services partners with individual physicians to coordinate the multiple therapies and services each patient may require - from skilled nursing care and infusion therapy to rehabilitative therapies and hospice care. Our patients also benefit at home from much of the same advanced technology available in Sentara hospitals.
We provide care to patients at the following locations:
Β Greater Hampton Roads, including Virginia Beach, Norfolk, Portsmouth and Chesapeake
Β Western Tidewater, including Suffolk, Surry, Sussex, Isle of Wight, Franklin, Smithfield, Wakefield, Ivor, Windsor, Zuni and surrounding areas
Β Greater Peninsula locations, including Williamsburg, Hampton, Newport News, Poquoson and Yorktown
Β Gloucester and surrounding areas
Β Charlottesville and the surrounding areas
Β Rockingham and the surrounding areas
Β Alleghany and the surrounding areas
Β Halifax and the surrounding areas
Β Northern Virginia, including Prince William County and surrounding areas
Β Northeastern North Carolina counties of Elizabeth City and surrounding areas

Job Summary
The Registered Nurse develops, implements and evaluates plans of care for specific patient populations. Coordinates and collaborates with multidisciplinary team members to facilitate integrated and comprehensive care. Possesses clinical knowledge and skills to meet standards as required by specific clinical areas.
Additional experience for specialty areas may be required. Nursing experience preferred.
Registered Nurse License (RN) - Nursing License - Compact/Multi-State License is required
Hospital at Home RN is only for SNGH Hospital at Home or SLH Hospital at Home
Qualifications:
N-2YR - RN-Associate's Degree (Required), N-3YR - RN-Diploma (Non-degree) (Required), N-4YR - RN-Bachelor's Level Degree (Required), N-6YR - RN-Master's Level Degree, N-DN - RN-Doctorate Level Degree
Basic Life Support (BLS) - Certification - American Heart Association (AHA) RQI, Registered Nurse (RN) Single State - Nursing License - North Carolina, Registered Nurse (RN) Single State - Nursing License - Virginia Department of Health Professionals (VADHP)
