Key Responsibilities:
- Conduct virtual assessments, monitor patient progress, and coordinate care plans with the healthcare team.
- Provide telehealth support to patients, including education on medications, therapies, and treatment plans.
- Collaborate with physicians, pharmacists, and other healthcare professionals to ensure optimal patient outcomes.

Qualifications:
- Current RN license in the state of practice.
