In this role, you will be instrumental in delivering exceptional patient care and ensuring the well-being and safety of patients at all stages of their healthcare journey. Your responsibilities will include assessing patient needs, crafting personalized care plans, administering medications, and monitoring progress.
Key Responsibilities
- Deliver direct patient care to diverse age groups and health conditions.
- Perform comprehensive assessments and maintain accurate patient histories.
- Develop and execute individualized care plans tailored to patient needs.
- Administer prescribed medications and treatments.
- Monitor vital signs and document any changes in patient conditions.
- Work collaboratively with interdisciplinary teams to ensure complete patient care.
- Educate patients and families on health management and disease prevention.
- Ensure all patient records and documentation are precise and current.
- Graduate of an accredited school of professional nursing. BSN, PHN preferred.
- Licensed driver with automobile insured in accordance with state and/or Agency requirements and in good repair. Good driving record.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Active RN license in the state of Arizona or a multistate license.
- Per Diem: Candidates applying for per diem must have 6 months of home health experience as an RN.
- Full-Time: No RN and home health experience is required.
