RN Case Manager- Spanish Bilingual
JOB DESCRIPTION
Job Title: Remote RN Case Manager-Bilingual in Spanish
Duration: 6 Months (Extension Possible)
Pay Rate: $48/hr (W2)
Location: 100% Remote (Open to all U.S. time zones; CST preferred for 1β2 candidates)
Work Authorization: Must be a U.S. Citizen
Job Overview:
We are seeking a skilled Remote RN Case Manager to join our team on a 6-month contract, with potential for extension. This role is focused on ensuring that members with complex medical or psychosocial needs receive high-quality, cost-effective care. The RN Case Manager will perform holistic assessments and collaborate with internal and external stakeholders to develop and implement individualized care plans.
Key Responsibilities:
β’ Conduct telephonic interviews using motivational interviewing techniques
β’ Collaborate with case management team, physicians, and interdepartmental staff
β’ Educate members using approved tools to support care plan adherence
β’ Collect and evaluate medical data to identify unmet needs
β’ Create and monitor individualized care plans
β’ Coordinate with providers, home health, and community resources
β’ Participate in interdisciplinary team meetings and quality initiatives
β’ Maintain accurate documentation and adhere to regulatory guidelines
β’ Remain informed about benefit design, cost analysis, and community resources
β’ Promote collaboration with external payers, agencies, and partners
β’ Participate in internal training and continuing education
Required Qualifications:
β’ RN License with Bachelorβs degree in Nursing (or equivalent RN certification)
β’ Minimum 3 years of experience in Acute or Managed Care (Medicaid/Medicare populations preferred)
β’ 3β5 years of experience in Acute Care/Managed Care settings
β’ Telephonic case management experience
Preferred Qualifications:
β’ Bilingual in English/Spanish
β’ Case Management Certification (CCM or similar)
