RN Case Manager
Contract and permanent positions available

Cambridge, Ontario (On-site, hybrid and remote positions available)
Ā Conducting daily monitoring of hospitalized patients and/or outpatient care, updating notes and keeping record of decisions in case management computer system.
Ā Secure Medical Consultants or Registered Nurses assessment of medical information.
Ā Provide approvals for defined procedures and follow-ups or liaises with medical team as required.
Ā Liaise with the patient and/or family regarding benefit information, itinerary, technical benefits and arrangements for the case.
Ā Review the clients policy and investigate the file to determine most appropriate course of action.
Ā Make billing arrangements (incl. coverage letters etc.) and obtain updated bill amounts.
Ā Coordinate the repatriation of the client, along-side the Transportation Services team.
Must Have:
Ā 2 year experience working as a Registered Nurse or RN Case Manager
Ā Bachelor of Science in Nursing
Ā Current certificate of competence as a Registered Nurse with the College of Nurses of Ontario
Ā Ability to work shifts consisting of days, evenings, nights, weekends and statutory holidays (shift premiums available).
Ā Current certification in Basic Cardiac Life Support BCLS.
