Key Responsibilities
Β Conduct thorough assessments to evaluate patient needs.
Β Develop tailored care plans that align with individual health goals.
Β Coordinate effectively with physicians, hospitals, and various healthcare providers.
Β Educate and guide patients and their families throughout their healthcare journey.
Β Advocate for patients to ensure they receive optimal care.
Β Maintain precise and updated patient records.
Required Skills
Β Current Registered Nurse (RN) license.
Β Minimum of 2 yearsΒ experience in case management, including managed care or home care.
Β Strong communication and interpersonal abilities.
Β Capacity to work independently as well as collaboratively within a team.
Β Excellent organizational and time management skills.
Β Bilingual skills (Spanish, Russian, or Chinese) are advantageous.
Qualifications
Β Valid and active Registered Nurse (RN) license.
Β At least 2 years of relevant experience in case management or a related field.
