[Remote] Vice President, HealthBridge Operations & Clinical Integration

The Vice President of HealthBridge Operations & Clinical Integration will lead the development and scaling of the service delivery model, transforming successful practices into a standardized national operating system.ResponsibilitiesDesign and operationalize the national HealthBridge care transition delivery modelTranslate successful field-based practices into standardized workflows, SOPs, and scalable operational processesDevelop implementation playbooks for hospital and health system partnershipsBuild operational infrastructure capable of supporting multi-market expansionPartner with hospital leadership, case management teams, discharge planners, and healthcare stakeholders to support successful integrationEstablish standardized onboarding and implementation protocols for new health system partnersDevelop escalation pathways and service recovery proceduresEnsure consistent operational execution across all marketsCreate standardized best practices for patient navigation, discharge coordination, and post-acute transition supportEnsure workflows align with hospital throughput goals, patient experience expectations, and quality metricsCollaborate with clinical and operational stakeholders to improve continuity of care and reduce transition frictionEstablish operational KPIs and service-level metricsDevelop dashboards and reporting frameworks for hospital partners and executive leadershipMonitor program effectiveness, operational consistency, referral responsiveness, and transition outcomesDrive continuous process improvement initiativesBuild and oversee HealthBridge operational training and certification programsDevelop implementation resources, communication protocols, and operational standards for field teamsPartner with franchise and regional leadership to support execution consistencyCollaborate closely with executive leadership, business development, franchise operations, technology, and clinical stakeholdersServe as a strategic operational leader during HealthBridge national expansionHelp shape long-term organizational strategy and scalable infrastructureSkills10+ years of progressive leadership experience in healthcare operations, care transitions, population health, post-acute care, or hospital operationsExperience building or scaling multi-site healthcare operational programsStrong understanding of hospital discharge planning, case management, care coordination, and post-acute ecosystemsProven success creating standardized workflows, SOPs, operational playbooks, and scalable service modelsExperience leading implementation initiatives across multiple markets or healthcare systemsStrong analytical and KPI-driven operational management experienceExperience working with hospitals, health systems, ACOs, Medicare Advantage organizations, or value-based care programsExperience in transitional care management, throughput operations, population health, or post-acute strategyFamiliarity with franchise, distributed workforce, or multi-market operating environmentsExperience in healthcare startups, growth-stage healthcare organizations, or healthcare services expansionBenefitsBase salaryPerformance incentivesBenefits packageGrowth and leadership opportunitiesCompany OverviewSilverAssist provides a suite of complementary services that connect seniors and their families with informative resources.
