Remote Telehealth Physician
Job Title: Contract Physician - Telehealth Services (Part-Time)
Location: Remote, Telehealth (Must hold California State License)
Position Overview:
Centered Care Wellness, a mission-driven healthcare organization dedicated to improving access to quality healthcare for vulnerable populations, is seeking a compassionate, part-time Contract Physician. This role will provide telehealth services to patients in California, focusing on chronic disease management, preventive care, and transitional care for adults and seniors. As part of a multidisciplinary team, you will play a critical role in supporting patients' health goals and reducing hospital readmissions.
Key Responsibilities:
- Provide remote, patient-centered care via telehealth, focusing on chronic disease management, preventive care, and transitional support.
- Evaluate and manage patient health, diagnose conditions, and develop treatment plans in alignment with patient goals.
- Collaborate with and, when necessary, supervise and support Advanced Practice Registered Nurses (ARNPs) and Physician Assistants (PAs).
- Oversee and guide home health care plans, ensuring continuity and quality of care for patients transitioning from hospital to home.
- Utilize electronic medical records (EMR) and telehealth platforms to maintain accurate documentation of patient encounters.
Qualifications:
- MD or DO, licensed to practice in California.
- Board certification in Internal Medicine, Family Medicine, Gerontology
- At least 2 years of experience in primary care or chronic disease management, preferably with vulnerable populations. Telehealth experience preferred.
- Bilingual (English/Spanish) is highly preferred.
- Strong communication skills and a patient-first approach, with a passion for accessible, empathetic healthcare.
