Remote Social Services Case Manager

Our client is looking for a compassionate and dedicated Remote Social Services Case Manager to join their fully remote team and make a significant impact in the community. This role is vital in providing support, resources, and advocacy to individuals and families facing various challenges. As a remote case manager, you will leverage technology to connect with clients, assess their needs, develop personalized service plans, and coordinate access to essential services. This position requires exceptional empathy, strong organizational skills, and a deep commitment to empowering vulnerable populations. The flexibility of a remote role allows for effective client engagement while maintaining a healthy work-life balance. Key responsibilities include: conducting comprehensive needs assessments with clients via virtual platforms; developing and implementing individualized case plans to address social, emotional, and practical needs; connecting clients with community resources, including housing assistance, healthcare, employment services, and mental health support; advocating on behalf of clients to ensure they receive appropriate services and benefits; monitoring client progress and adjusting service plans as needed; maintaining accurate and confidential client records in compliance with privacy regulations; collaborating with other service providers, agencies, and stakeholders; providing emotional support and counseling to clients and their families; facilitating client participation in support groups or therapeutic activities; and staying informed about available community resources and legal/policy changes affecting service delivery. A Bachelor's degree in Social Work, Psychology, Sociology, or a related human services field is required. A Master's degree and licensure (LCSW, LMSW, LPC, etc.) are preferred. At least 3 years of experience in case management or social work is essential. Excellent communication, active listening, problem-solving, and digital literacy skills are crucial for success in this fully remote position. Experience with telehealth platforms and case management software is a plus.
