[Remote] Senior Analyst, Health Coach Wellness

They are seeking a Senior Analyst, Health Coach Wellness to telephonically engage with the Medicare population, improving medication adherence and health outcomes through education and support. The role involves a collaborative process to assess and promote members' health decisions and provide guidance on available healthcare services.ResponsibilitiesAssessment of Members: Through motivational interviewing and the use of various tools, uses healthcare knowledge and experience of health risk behaviors to conduct evaluation of member's health status for either HEDIS health screening/control or Medication AdherenceUtilizes influencing/motivational interviewing skills to ensure member engagement and discern health needs based on key questions and conversation to promote behavior changes to achieve optimum level of healthPartners with members to identify opportunities for integration into available internal/external programsUsing holistic approach consults with Manager or others to overcome barriers with assisting members in meeting goals and objectivesUtilizing interviewing and active listening skills, provides coaching, information and support to empower the member to make ongoing independent healthy healthcare choicesInterprets and utilizes guidelines/criteria to positively impact members healthProvides up-to-date healthcare information to help facilitate the member’s understanding of his/her health statusHelps member actively and knowledgably participate with their provider in healthcare decisionsApplies judgment to the incorporation of strategies designed to reduce risk factors and barriers and address complex health and social indicators which impact healthcare access, services and resolution of member issuesDevelops a proactive course of action to address issues presented to enhance the short and long term health outcomes as well as opportunities to enhance a member’s overall wellness through integrationSkills1-3 years Microsoft Office3-5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, OncologyManaged Care and Medicare experienceAssociates Degree or equivalent experienceHealthcare licensure and/or certifications (RN, LPN, MSW, LCSW, CDE etc) highly preferred5 years practice experience-- Renal, Diabetes, Hypertension, Primary Care/Geriatric, OncologyPrevious telephonic coaching, education or case management experienceExcellent computer skills including MS Office applicationsIndependent workerExcellent time managerExcellent analytical and problem solving skillsEffective communications, organizational, and interpersonal skills, Bilingual a plusDemonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in challenging situationsExperience in facilitating client positive behavioral changeBackground in motivational interviewing, active listening skillsBenefitsThis position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.This full‑time position is eligible for a comprehensive benefits package designed to support the physical, emotional, and financial well‑being of colleagues and their families.The benefits for this position include medical, dental, and vision coverage, paid time off, retirement savings options, wellness programs, and other resources, based on eligibility.Company OverviewCVS Health is a health solutions company that provides an integrated healthcare services to its members.
