Key Responsibilities:
Deliver expert health education and coaching to members, enabling them to navigate the healthcare system with confidence
Facilitate seamless transitions of care by coordinating services, referrals, and appointments
Conduct thorough assessments to identify gaps in care and develop targeted interventions to address them
Provide empathetic support and guidance to members, addressing their unique needs and concerns
Cultivate strong relationships with members, providers, and internal stakeholders to drive positive health outcomes
Utilize advanced clinical skills, critical thinking, and problem-solving to address complex member needs
Requirements:
Current, unrestricted RN licensure
3+ years of clinical RN experience, with a strong foundation in clinical skills and critical thinking
Proven expertise in telephonic communication, with excellent inbound and outbound skills
Intermediate computer skills, including Microsoft Word, Outlook, and Internet navigation
Ability to work in a fast-paced, dynamic environment, with a focus on delivering exceptional member experiences
Preferred Qualifications:
Bachelor's degree in Nursing
Coaching and/or decision support experience
Managed care experience
Conflict management skills
Proven understanding of change management principles and the ability to motivate members towards positive health behaviors
