Develop and oversee personalized treatment plans for patients with musculoskeletal, neurological, or other disabling conditions.

Monitor progress and adapt care plans to achieve optimal outcomes.

Provide adaptive tools and rehabilitation strategies to enhance independence and well-being.

Counsel patients and families on lifestyle adjustments, rehabilitation goals, and long-term care needs.

Collaborate with facility staff, nursing teams, and other providers to deliver integrated, high-quality care.

Serve as a trusted advocate for patients and a clinical leader within partner facilities.
