Description
The Telehealth Pharmacist will provide high touch tele-health services which include, but are not limited to Medication Therapy Management (MTM), basic medication education, referrals to support services, etc.
Role Breakdown
MTM related services Ā
Ā Counseling participants on the importance of medication adherence (alerting participants to missed dosages and refills)
Ā Providing medication education
Ā Providing self-care education for specific chronic conditions
Ā Contacting physicians to schedule diagnostic testing
Ā Contacting physicians to make medication therapy recommendations
Ā Connecting with and supporting Community Health Workers advocating for the participant.
Ā Connecting participants with our community-based resources as needed.
Program Documentation Ā
Pharmacist Will Need To Maintain Proper Documentation Of Each Service Provided To Our Patient, And Program Documentation For Each Service Should Include The Following Information
Ā First name, Last Name, and Date of Birth
Ā Patient Signature or retrievable electronic verification of services received
Ā Date the service was provided (month/day/year)
Patient Coordination Ā
Ā Coordinating a day/time to conduct MTM-related services to patients
Ā Coordinating patient signatures directly after service
Ā Coordinate and communicate with In-Store pharmacy staff about patient related topics

The pay range for this position is $50.00 - $55.00
