Are you a Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) passionate about proactive, patient-centered care?

Nsight Health, Inc. is seeking a Remote RPM Nurse to join our dynamic Contact Center team. In this role, you'll monitor and manage patient alerts, conduct proactive outreach to improve outcomes, and provide education to support ongoing care. You'll also handle both inbound and outbound calls, routing them based on clinical urgency and patient needs.

Key Responsibilities
β€’ Proactively manage high volumes of outbound patient calls
β€’ Monitor and respond to RPM alerts with appropriate follow-up
β€’ Conduct monthly wellness assessments and chart reviews
β€’ Coordinate care and escalate issues to healthcare providers
β€’ Accurately document interactions in our clinical software
β€’ Meet performance metrics and patient engagement goals
β€’ Support a compassionate, team-focused remote work environment

What We’re Looking For
β€’ Active LPN, LVN, or RN license (Required)
β€’ Strong empathy, communication, and patient education skills
β€’ Moderate to advanced computer literacy
β€’ Reliable, high-speed internet and HIPAA-compliant workspace (speed test required during interview)
