[Remote] Patient Disability Advocate

This role involves conducting eligibility screenings, assessing patient financial requirements, and acting as a liaison between patients, hospitals, and governmental agencies to assist disabled patients in obtaining benefits.

Responsibilities
• Screen and evaluate patients for existing insurance coverage, federal and state assistance programs, or hospital charity applications
• Obtain legally relevant medical evidence, physician statements and all other documentation required for eligibility determination
• Complete and file applications. Initiate and maintain proper follow-up with the patient and government agency caseworkers to ensure timely processing and completion of all mandated applications and accompanying documentation
• Assist the patient with gathering any additional verifications or medical records and meeting appointment dates and times. Maintain structured and timely contact with the patient and agency as outlined in the company standards
• Provide advocacy work for patients seeking disability benefits through the Social Security Administration (SSI/SSDI) programs and assists with opening claims and assisting with pending claims for the Social Security Administration
• Ensure all insurance, demographic and eligibility information is obtained and entered into the system accurately. Document progress notes to the patient’s file and the hospital computer system

Skills
• Minimum one-year related experience required, preferably in healthcare
• Relevant education may substitute experience requirement
• Work from home employees are required to have high speed internet 25 MB download and 6 MB upload
