Remote Patient Care Coordinator

Responsibilities:
Schedule nurses and Home Health Aides (HHAs) to meet patient needs.
Utilize intermediate to advanced proficiency with Microsoft Office Suite (Word, Outlook, Excel).
Participate in a weekend on-call rotation.
Communicate shift confirmations to the care team.
Manage trackers and spreadsheets to ensure efficient scheduling and resource allocation.
Monitor nurse and HHA visits, gathering patient satisfaction feedback through calls.
Employ various technology platforms for tracking, time management, and report generation.
Collaborate with different departments to address diverse patient needs.
