A forward-thinking telehealth practice is seeking a Remote Nurse Practitioner (NP) to deliver chronic care and lifestyle medicine via virtual consultations. Providers will manage patient health through data-driven care, collaborate with a multidisciplinary team, and empower patients with wellness education.
