[Remote] Nurse Consultant (Clinical Leadership and MDS experience)

They are seeking a Nurse Consultant to provide on-site consulting services, conduct compliance audits, and support performance improvement initiatives while traveling nationwide.

Responsibilities
• Provide on-site consulting services nationwide (75–80% travel)
• Conduct clinical compliance audits and mock surveys
• Perform Medicare compliance and MDS documentation audits
• Evaluate MDS accuracy and supporting documentation under PDPM
• Provide corrective action guidance and performance improvement support
• Submit reports and maintain timely communication with leadership
• Remain current on federal and state regulatory requirements

Skills
• Active Registered Nurse license
• Minimum five years long-term care experience
• Experience in clinical compliance and regulatory oversight
• Working knowledge of MDS and Medicare reimbursement
• Strong communication and organizational skills
• Ability to travel nationwide 75–80%
• Detail-oriented individuals who are team players
• Adaptable to change and willingness to learn different processes
• Polaris requires employees working from home, or otherwise remote, maintain broadband internet access at the address where remote work is being performed
• Ability to function well in a fast-paced and at times stressful environment
• Prolonged periods of sitting at a desk and working at a computer
• Ability to lift and carry items weighing up to 10 pounds at times
• Experience as DON, Regional Nurse Consultant, MDS Coordinator, or state surveyor
• RAC-CT certification
• Bachelor of Science in Nursing degree
