Job Overview

Join our dedicated team as a Medical Management Specialist, a role designed for compassionate and skilled professionals eager to make a meaningful impact in healthcare. This position is fully remote, allowing you to deliver essential medical management and health education services to individuals enrolled in government healthcare programs. As a key contributor, you will gather, analyze, and disseminate data for regulatory reporting while performing assessments, crafting individualized care plans, and coordinating care with members and their families, both in-person and through virtual communication.

Key Responsibilities
• Facilitate medical management and health education initiatives for individuals within government healthcare programs.
• Collect and analyze pertinent data to generate reports for regulatory compliance.
• Execute assessments and provide care coordination through telephonic and face-to-face interactions with members and their families.
• Design tailored care plans that meet the specific needs of individuals.
• Represent the organization with professionalism during all interactions with members, healthcare providers, and community resources.
• Engage in occasional travel within your designated territory as required.

Required Skills
• Excellent verbal and written communication abilities, enabling effective discussions regarding medical needs with members and collaborations with internal teams and external partners.
• Strong analytical skills with experience in reviewing and interpreting medical data.
• Proficiency in Microsoft Office Suite, including Word, Excel, and PowerPoint, as well as familiarity with web-based applications.
• Willingness and ability to travel within the assigned region.

Qualifications
• Valid and unrestricted Registered Nurse (RN) license with a minimum of three years of direct clinical care experience, OR
• Licensed Professional Counselor (LPC) or Licensed Master Social Worker (LMSW) with at least two years of clinical experience.
• A current, valid, unrestricted license to practice in the state of employment (or eligibility for reciprocity).
• Minimum of three years of experience in wellness or managed care, demonstrating the ability to present clinical issues to members or healthcare providers.
• In-depth knowledge of the health and wellness industry, including current trends in employer healthcare offerings.
