Salary: $65,000
Duties and Responsibilities:
ā¢ Provide enhances post release services for UC who would benefit from intensive, clinical case management services.
ā¢ Encourages children served to express their feelings and discuss what is happening in their lives, and help them to develop insight into themselves and their relationships.
ā¢ Guides children served in the development of skills and strategies for dealing with their problems.
ā¢ Prepares and maintain all required treatment records and reports.
ā¢ Counsels children, individually and in group sessions, to assist in overcoming dependencies, adjusting to life, and making changes.
ā¢ Acts as client advocates in order to coordinate required services or to resolve emergency problems in crisis situations.
ā¢ Develop and implement treatment plans based on clinical experience and knowledge in conjunction with children served.
ā¢ Collaborates with other staff members to perform clinical assessments and develop treatment plans.
ā¢ Evaluates children's served physical or mental condition based on review of client information.
ā¢ Meets with families, probation officers, police, and other interested parties in order to exchange necessary information during the treatment process.
ā¢ Refers children served, or family members to community resources or to specialists as necessary.
ā¢ Counsels family members to assist them in understanding, dealing with, and supporting children served.
ā¢ Modifies treatment activities and approaches as needed in order to comply with changes in children served status.
ā¢ Discusses with individual patients their plans for life after leaving therapy.
ā¢ Gathers information about community mental health needs and resources that could be used in conjunction with therapy.
ā¢ Monitors children served use of medications.
ā¢ Assess the safety and appropriateness of the placement on an ongoing basis using child welfare principles and social work best practices and conduct calls, interviews, and home visits.
ā¢ Build a network of local pro bono legal resources, food and clothing banks, mental health services and care providers to establish services for youth.
ā¢ Participate in regular training and communication sessions with on-site and case work supervisor and headquarters staff.
ā¢ Position requires up to 50 percent of in state and/or out of state travel with limited notice.
ā¢ Ability to perform duties and services remotely from home.
Qualifications:
Required:
ā¢ Minimum of a Master's Degree in Psychology, Sociology, or other relevant behavioral science in which direct clinical experience is a requirement, commensurate experience in relevant field may be acceptable in lieu of degree
ā¢ Three years or more years of working with the program population.
ā¢ Bilingual in English/Spanish.
Preferred:
ā¢ Master's degree in Psychology, Sociology, or other relevant behavioral science.
ā¢ Five years or more years of working with the program population.
