[Remote] In-Home Dialysis Care Partner (1:1 Client) – Fluent Spanish Required

Herewith GmbH is seeking a compassionate and reliable Care Partner fluent in Spanish to support in-home dialysis for a patient transitioning from clinic to home care. The role involves providing safe and high-quality dialysis support, building trust with the patient, and ensuring a smooth care transition. Responsibilities Assist with in-home dialysis treatment setup (NxStage or similar systems) Maintain a clean, organized, and safe treatment environment Support the patient in staying on schedule with dialysis treatments Provide companionship and emotional reassurance Assist with light Activities of Daily Living (ADLs) as needed Monitor patient safety and report concerns to the coordinating nurse Maintain simple documentation and shift notes Skills Minimum 1 year of dialysis experience (home or clinic) Fluent in Spanish (required) Open to in-home dialysis care and completing provided training Calm, patient, and dependable—especially during care transitions Strong communication skills and ability to collaborate with a clinical team Must pass a Herewith background check Familiarity with home dialysis systems (NxStage a plus) Benefits Comprehensive training will be provided, including patient-specific dialysis procedures, home setup guidance, and continued support from our clinical team Consistent, stable weekly hours with a single client Structured training and patient-specific guidance Ongoing support from an experienced clinical care team Independence in your role while remaining part of a dedicated care network Company Overview Herewith Inc.
