Key Responsibilities
ā¢ Conduct assessments to determine the social, emotional, and financial needs of patients and their families.
ā¢ Develop and implement care plans that address the unique challenges faced by patients in hospice care.
ā¢ Offer emotional support and counseling to patients and families coping with end-of-life issues.
ā¢ Collaborate with healthcare teams to ensure coordinated and comprehensive care.
ā¢ Educate families about available resources and services, and assist them in navigating the healthcare system.
Required Qualifications
ā¢ A Master's degree in Social Work (MSW) from an accredited institution.
ā¢ Current state licensure as a clinical social worker.
ā¢ Experience in hospice or palliative care settings is preferred.
ā¢ Strong communication and interpersonal skills, with a genuine compassion for patients and their families.
ā¢ Ability to work independently in a remote setting while staying connected with the team.
