Remote Home Health Intake LVN Coordinator - Join Our Team!

As a key member of our Sutter Care at Home team, you will play a vital role in managing new referrals, facilitating timely admissions, and providing exceptional customer service to patients, families, and referral sources.

Key Responsibilities:
Manage new referrals and ongoing follow-ups, ensuring timely and consistent admissions
Communicate effectively with patients, families, and referral sources, maintaining clear and concise communication
Collaborate with referring hospitals, physicians, board and care facilities, skilled nursing facilities, and other healthcare providers
Enter referral information into the Electronic Medical Records (EMR) system and create initial orders to facilitate the start of care
Utilize and promote excellent customer service skills in all interactions
Collaborate with referral center staff, home health, Advanced Illness Management (AIM), and hospice staff across offices
Requirements:
Graduate of an accredited school of Vocational Nursing
Unencumbered LVN-License Vocational Nurse license for the state you reside
Must have or obtain an LVN-License Vocational Nurse of California within 3 months of hire (company-sponsored and assisted)
1 year of recent relevant experience
