As part of our commitment to innovation and excellence, we are expanding our Chronic Care Management (CCM) program and are seeking a Registered Nurse (RN) to support our care management team remotely.If you have a passion for patient care and experience in chronic care management, we’d love to hear from you!Key Responsibilities:
Assist patients by providing information, coordinating services, and offering support remotely.
Conduct outreach to patients for follow-ups, wellness checks, and care coordination.
Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems.
Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions.
Educate patients on managing chronic conditions and adhering to treatment plans.
Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
Stay updated with industry best practices in telehealth and chronic care management.
Qualifications:
Registered Nurse (RN) license in New York State
2-3 years of experience in a medical office, preferably in chronic care management or telehealth
Proficiency in EMR systems and medical terminology
Strong verbal and written communication skills
Bilingual in Spanish highly preferred
Experience working in a remote healthcare setting is a plus
Compensation & Benefits:
Pay Rate: $38-48 per hour
