Remote Entry-Level Nursing Role - Care Management Opportunity

As part of our commitment to innovation and excellence, we are expanding our Chronic Care Management (CCM) program and are seeking a Registered Nurse (RN) to support our care management team remotely.

Key Responsibilities:
? Assist patients by providing information, coordinating services, and offering support remotely.
? Conduct outreach to patients for follow-ups, wellness checks, and care coordination.
? Maintain and update medical records, ensuring accuracy in electronic medical records (EMR) systems.
? Support physicians and care managers by relaying patient updates, scheduling appointments, and ensuring smooth care transitions.
? Educate patients on managing chronic conditions and adhering to treatment plans.
? Maintain patient confidentiality and comply with HIPAA and healthcare regulations.
? Stay updated with industry best practices in telehealth and chronic care management.
Qualifications:
? Registered Nurse (RN) license in New York State
? 2-3 years of experience in a medical office, preferably in chronic care management or telehealth
? Proficiency in EMR systems and medical terminology
? Strong verbal and written communication skills
? Bilingual in Spanish highly preferred
? Experience working in a remote healthcare setting is a plus
Compensation & Benefits:
? Pay Rate: $38-48 per hour
