Job Overview

We are seeking a dedicated and highly skilled Remote Emergency Department (ED) Case Manager to join our healthcare team. This vital role involves coordinating patient care from emergency departments across various healthcare facilities, ensuring seamless transitions, appropriate resource utilization, and comprehensive discharge planning. The ideal candidate will possess extensive experience in emergency medicine, critical care, and hospital-based case management, with a strong understanding of acute care processes and patient assessment. Responsibilities
• Conduct thorough patient assessments by reviewing medical records, vital signs, and diagnostic evaluations to determine care needs.
• Collaborate with emergency room teams, physicians, and multidisciplinary providers to develop individualized care plans aligned with clinical protocols.
• Coordinate post-acute care services, including discharge planning, outpatient follow-up, and referrals to specialized providers such as behavioral health or geriatrics.
• Monitor utilization management to ensure appropriate level of care while adhering to HIPAA regulations and medical terminology standards.
• Facilitate communication between hospitals, primary care providers, and community resources to optimize patient outcomes.
• Maintain accurate documentation within medical records systems and ensure compliance with healthcare regulations.
• Support diagnostic evaluation processes and provide case management for inpatient and outpatient populations across diverse settings.
• Engage in ongoing patient service efforts by providing education on treatment plans and post-discharge instructions.
• Participate in quality improvement initiatives focused on patient safety, discharge efficiency, and resource utilization.

Experience
• Proven experience in emergency room settings or hospital medicine environments with a focus on acute care case management..
• Experience working within managed care organizations or utilization management programs.
• Strong knowledge of physiology, medical terminology, and hospital-based patient assessment techniques.
• Prior outpatient clinic experience or inpatient hospital experience involving medical-surgical units is advantageous.
• Excellent communication skills combined with a comprehensive understanding of HIPAA compliance standards.
• Ability to navigate electronic medical records efficiently and maintain meticulous documentation.
