[Remote] ECM Clinical Manager (LCSW/LMFT)

The ECM Clinical Manager will oversee remote clinical management of Enhanced Care Management staff, ensuring clients receive appropriate medical and behavioral health services while collaborating with various care team members. This role requires supervising a team, managing care plans, and providing clinical oversight, with some travel required for training and meetings.ResponsibilitiesSupervise a team of approximately 10-15 direct reports under the ECM team to ensure high-quality enrollments and care management services. The exact number of direct reports may fluctuate based on business needsOversee client outreach, enrollments, and the creation of Individual Care Plans, assessing if clients' needs can be managed by an ECM Lead Care Manager; serve as an escalation point when necessaryDrive the team to meet key performance indicators (KPIs) effectivelyCollaborate with the Director(s) of Enhanced Care Management to implement changes and process improvementsProvide clinical oversight approximately 30% of the time, including, but not limited to: supervision of clinical staff, clinical sign-offs, oversight of care plans, leading IDT meetings, and serving as a clinical consultant for complex clinical casesProvide direct care management as needed to support organizational goalsTravel up to 20-25% may be required for training, meetings, and other business needsSkillsActive, unrestricted LCSW or LMFT license in California requiredWillingness to obtain additional state licensure as needed2 years of people management experience2-3 years of care management experience1-2 years of post-acute experienceProven, results-driven leader with experience in change management in rapid-growth environmentsStrong organizational and independent working skillsProficient in Microsoft and Google applications; Salesforce experience is a plusExcellent communication and motivational skills, including conflict resolution techniquesWilling and able to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends4 years preferredWorking knowledge of lean Six Sigma methodology is preferredBenefitsMedical, dental, and vision insuranceIncome protection benefitsFlexible PTOCompany holidays401kAccess to other wellness benefitsCompany OverviewVynca is a health technology and services company transforming care for people living with serious illness and complex needs.
