[Remote] District Supervisor, Innovations Waiver - Medicaid and State Southern Region

They are seeking a District Supervisor for the Innovations Waiver to oversee a team providing support for members with Intellectual or Developmental Disabilities and Traumatic Brain Injury, ensuring they receive whole-person care.ResponsibilitiesProvide direct supervision and oversight for assigned team of localized, multi-disciplinary team supporting members with intellectual/developmental/TBI disabilitiesEnsure staff utilize workflows as required in Trillium Connections to complete Care Management comprehensive assessment and care plans for members as care needs changeEstablish a team based, person-centered approach to coordination of care to be implemented by all team members that effectively manages a member's physical health, behavioral health, and social determinant of health needs using established workflows and processes effectively and efficiently and in accordance with NCQA proceduresMonitor teams to help ensure integration of care through the establishment of a multi-disciplinary team (e.g. member, caretaker/legal guardian, PCP, behavioral health and IDD providers, specialists, nutritionists, pharmacy, etc.) to meet the member's needsSkillsExperience in a managed care environmentKnowledge of Intellectual or Developmental Disabilities or Traumatic Brain InjuryAbility to provide direct supervision and oversight for a multi-disciplinary teamExperience in implementing industry standard, evidence-based and best practicesFamiliarity with care management comprehensive assessment and care plansAbility to establish a team-based, person-centered approach to coordination of careKnowledge of NCQA proceduresExperience in monitoring teams for integration of careBenefitsA collaborative, mission-driven work environmentCompetitive benefits and work-from-home options for most positionsOpportunities for professional growth in a diverse inclusive cultureCompany OverviewTrillium Health Resources is a local governmental agency for individuals with serious mental health, and intellectual disability services.
