Job Summary:
We are seeking a compassionate and detail-oriented Clinical Support Care Coordinator to join our team. As a Care Coordinator, you will play a vital role in providing exceptional support to our members, addressing their benefit questions, care coordination needs, and program inquiries. Key Responsibilities:

Respond to incoming calls from members, addressing their concerns, and providing timely support and guidance
Accurately document member interactions in our designated PHI system, ensuring seamless communication and continuity of care
Manage an assigned member caseload, providing short-term care coordination and health navigation to ensure optimal outcomes
Collaborate with our interdisciplinary care team to facilitate effective communication, identify potential risks, and improve quality and cost outcomes
Conduct outreach calls to members to confirm services and ensure their needs are being met
Perform additional duties as assigned, demonstrating flexibility and a commitment to delivering exceptional results


Requirements:

Certified Medical Assistant (CMA), Certified Nursing Assistant (CNA), Registered Medical Assistant (RMA), or relevant medical training or work experience
High School diploma or equivalent
Proven ability to communicate effectively, both verbally and in writing, with excellent grammar, spelling, and punctuation skills
Experience working in a fast-paced environment, handling multiple priorities simultaneously


Preferred Qualifications:

Associate's Degree
Prior experience in a call center, care management, or healthcare environment, with familiarity with medical terminology and benefit products
Excellent telephone, organizational, and customer service skills
Team-oriented, organized, and detail-focused individual

Competitive salary range: $41,101 - $62,400 (dependent on location and experience)
