This remote position allows you to work from the comfort of your home while making a significant impact in the lives of individuals in the Beverly, Gloucester, Lynn, Danvers, Salem, and... Peabody areas.

Key Responsibilities
Ā• Evaluate individual needs and devise customized care plans.
Ā• Facilitate connections to vital resources, including social services, community support programs, and financial assistance.
Ā• Advocate for members to ensure their preferences and requirements are prioritized by the healthcare team.
Ā• Maintain clear and continuous communication with all relevant stakeholders to support effective care coordination.

Qualifications
Ā• Current and unrestricted Massachusetts RN license.
Ā• Minimum of 2 years of clinical experience.
Ā• Proficiency in Microsoft Office Suite (Word, Excel, Outlook).
Ā• Willingness to travel within the designated region to conduct visits with members.
