Remote Case Manager, Social Services

Our client is seeking a compassionate and dedicated Remote Case Manager to join their Community and Social Care division. This is a fully remote position, enabling you to make a tangible difference in the lives of individuals and families from your home office. You will be responsible for assessing client needs, developing individualized service plans, and connecting clients with essential resources and support services. The ideal candidate is empathetic, organized, and possesses strong advocacy and problem-solving skills. Key Responsibilities: Conduct comprehensive assessments of client needs, including physical, mental, social, and environmental factors. Develop, implement, and monitor individualized service plans in collaboration with clients and their families. Connect clients with appropriate community resources, including healthcare, housing, employment, education, and financial assistance programs. Provide ongoing support, guidance, and advocacy to clients throughout their service journey. Maintain accurate and confidential client records, documenting all services provided and progress made. Collaborate with a multidisciplinary team of social workers, healthcare providers, and community agency representatives. Monitor client progress towards achieving their goals and make adjustments to service plans as needed. Educate clients and their families about available resources and services. Respond to crisis situations and provide immediate assistance or referrals. Stay informed about community resources, eligibility requirements, and service delivery models. Ensure compliance with agency policies, procedures, and relevant regulations. Participate in case conferences, team meetings, and professional development opportunities. Qualifications: Bachelor's degree in Social Work, Psychology, Sociology, Human Services, or a related field. Master's degree is a plus. Minimum of 3 years of experience in case management or social services, working directly with diverse populations. Strong understanding of social service systems, community resources, and available support programs. Excellent assessment, interviewing, and active listening skills. Proficiency in developing and implementing effective service plans. Strong advocacy skills with the ability to navigate complex systems on behalf of clients. Exceptional organizational and time management skills, with the ability to manage a caseload effectively. Proficient in using case management software and electronic health records (EHR) systems. Excellent written and verbal communication skills. Empathy, patience, and a genuine desire to help others. Ability to work independently and collaboratively in a remote team environment. Knowledge of relevant laws and ethical considerations in social work practice.
