About the Role

Our client, a leading provider of accessible behavioral health services, is seeking a dedicated and licensed Remote Case Manager to join their fully virtual team, supporting individuals in Grand Rapids, Michigan and surrounding areas. This position allows you to provide essential case management services entirely from your home office, utilizing advanced telehealth technology to connect with clients and facilitate their access to care. You will be responsible for assessing client needs, developing comprehensive treatment plans, coordinating services with various providers, and advocating for client well-being. The ideal candidate is highly organized, compassionate, proficient in telehealth platforms, and committed to improving the lives of those facing mental health and substance use challenges. This role emphasizes autonomy and the ability to manage a caseload effectively within a supportive, remote-first environment.
Key Responsibilities Conduct comprehensive remote assessments of clients' needs, including mental health, substance use, and social determinants of health. Develop, implement, and monitor individualized case management and treatment plans. Coordinate client care with psychiatrists, therapists, primary care physicians, and community resources. Provide crisis intervention and support via phone, video conferencing, and secure messaging. Assist clients in navigating healthcare systems, accessing benefits, and obtaining necessary resources. Maintain accurate, timely, and confidential electronic health records (EHR) in compliance with HIPAA. Educate clients and their families about treatment options and available support services. Advocate for clients to ensure they receive appropriate and effective care. Collaborate effectively with a remote multidisciplinary team. Requirements Master's degree in Social Work, Psychology, Counseling, or a related field. Active and unrestricted state licensure in Michigan (e.g., LMSW, LPC, LMFT) or a relevant credential. Minimum of 3 years of experience in case management, mental health, or substance abuse services. Demonstrated proficiency with telehealth platforms, EHR systems, and other virtual communication technologies. Strong knowledge of mental health and substance use disorders, treatment modalities, and community resources. Excellent assessment, crisis intervention, and problem-solving skills. Exceptional verbal, written, and interpersonal communication skills. Ability to work independently, manage time effectively, and maintain professional boundaries in a remote setting.
