Remote Case Manager

Piper Companies is currently seeking a Case Manager, RN for a fully remote opportunity with a highly rated, quality-focused health insurance provider. This role serves as a critical bridge between healthcare delivery and medical coverage, advocating for patients and helping improve their overall quality of care and life. Candidates must hold an active North Carolina or compact RN license and be located in either North Carolina, South Carolina, Tennessee, Georgia, Alabama, or Florida.

Responsibilities of the Case Manager:
• Collaborate with a multidisciplinary healthcare team to monitor utilization patterns and facilitate appropriate care delivery for selected members.
• Conduct comprehensive assessments of members’ health status, including clinical history, medical records, support needs, and cultural considerations.
• Provide ongoing support and follow-up to ensure members’ care plans are progressing appropriately.
• Coordinate with primary care providers, medical directors, discharge planners, care coordinators, and family members to develop and implement treatment and discharge plans.
• Maintain accurate documentation and ensure compliance with care standards and protocols.

Qualifications of the Case Manager:
• Active RN license (North Carolina or compact state required)
• 3+ years of inpatient care experience, including critical care (e.g., Med-Surg, ICU)
• Prior telephonic case management experience is a plus
• Must reside in the Eastern Time Zone

Compensation & Benefits of the Case Manager:
• Hourly Rate: $36–$38/hr (based on experience), paid weekly
