Your Role
• Provide monthly calls and support to patients enrolled in the CCM program.
• Utilize your nursing skills to educate patients and their families.
• Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
• Active LVN license (CALIFORNIA)
• Experience in geriatrics or primary care
• Ability to sit and talk on the phone for long periods of time
• System: We require you to have a Windows i5 (or newer) processor, and to have Windows Pro (not Home). This can be a laptop or desktop.
• You live in any of the following states: CA, NV, NM, AZ, CO, UT, WA, OR
The Pay
• The California LVN pay starts at $28.85 per hour
• The pay structure is a base pay plus a bonus per call. On average our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
