What to Expect (Job Responsibilities):
- Provide resource support to members and connect them to local resources for various services
- Assist in assessing member needs and developing care plans/service plans
- Connect members to local resources, obtain required documentation, and support the application process
- Monitor delivery of care/services and perform follow-ups to ensure timely access
- Work with providers and community stakeholders to support members in accessing essential care/services
What is Required (Qualifications):
- Bachelor's degree and 2 4 years of related experience
- Ability to develop, assess, and adjust care plans
- Excellent coordination and communication skills
- Active participation in team care management rounds
- Risk identification and management skills

- Experience in a similar role in a healthcare or social services setting
