They are seeking an Internal Medicine or Family Medicine physician to provide high-quality clinical care, perform patient evaluations, and coordinate care plans in a telehealth setting.ResponsibilitiesJoin our medical group as an independent contractorProvide high-quality clinical care utilizing best practices and policies set forth by Carda’s clinical leadership teamPerform asynchronous or synchronous visits with patients on a schedule that suits your schedulePerform health histories, evaluate patient records, diagnose, and refer to our programs when appropriateProvide lifestyle recommendations to patients related to specific goalsInput notes and all other relevant information into our EMRCommunicate and partner with other members of the patients’ care team to properly coordinate care and oversee patient care plans on an ongoing basisJoin in occasional meetings and participate in a QA programSkillsInternal Medicine or Family Medicine physicianBoard certified in Family Medicine, Internal Medicine, Cardiology, PulmonologyIdeally licensed in five or more of the states. Priority states include: Florida, Pennsylvania, Virginia, Texas, Maryland, Illinois, Ohio, Michigan, Washington, Massachusetts, North Carolina, CaliforniaClean, or relatively clean record to facilitate ease of health plan credentialingNot currently opted out of MedicareWilling to complete credentialing applications in a timely matterExpertise in the management of individuals with cardiac and pulmonary pathophysiologyCardiopulmonary training in basic life support or advanced cardiac life supportPassion for delivering high quality care to patientsExperience with care in a virtual setting and a desire to learn and influence new technologiesHighly organized with the ability to create, adhere to, and scale repeatable processesLicensed In The Following States: Alabama, Arkansas, Georgia, Iowa, Louisiana, Mississippi, South Carolina, West VirginiaWilling to obtain additional licenses by mutual consent and with financial supportCompany OverviewCarda Health provides virtual cardiac and pulmonary rehabilitation services for patients.
